Often, when filling an OSAP application, if you are a single parent of your children, you will be asked to provide a proof, by way of an affidavit, that you are a single mother or parents of your children and the kids will live with you for the duration of your program. 

 

This affidavit is an important document, which needs to be sworn and commissioned in front of a Commissioner of Oaths etc. in the Province of Ontario before you could submit it to the OSAP.
